The mission of the Children's Attention Home is to provide nurturing care and life-changing services for youth and families in need. Every person on our 60+ member team has an important role that leaves a lasting impact on the lives of those we serve.

As a provider of residential services to children who have experienced situations of abuse, neglect, or homelessness, we must take the utmost care when reviewing candidates. Hiring policies are taken directly from the state and federal standards under which we operate. All candidates must agree to a SC Central Registry Check, SLED check, and fingerprinting.

Position Summary and Purpose:

The Care Coordinator serves as the primary cottage leader in the oversight and supervision of team members and the provision of care. Care Coordinator are responsible for case management of residential services and provision of those services deemed necessary to insure the health, safety, legal welfare, educational, emotional and spiritual well-being of residents in care.

Primary Responsibilities:

  • Serves as a member of the intake team responsible for ensuring that all necessary paperwork is completed for new admissions.
  • Screens resident's application/referrals for appropriateness of placement and provides assistance and/or conducts pre-placement interviews and resident exit interviews.
  • Coordinates the development and implementation of Comprehensive Care Plans for each resident, including a daily behavior modification program.
  • Facilitate weekly staffing of residents in conjunction with Residential Counselors, Independent Living Coordinator, Social Worker, and Educational Coordinator to discuss specific residents' cases.
  • Spend at least 1 hour per week with residents assigned to caseload in reviewing plans, monitoring peer interactions, and daily living skills.
  • Coordinates the provision of residential services such as, recreational activities, referrals for health care, psychological evaluations and therapeutic counseling.
  • Attend school and parent conferences, IEP/resource/progress meetings, and other educational events along with the Educational Coordinator.
  • Assist in identifying the individual educational needs of residents in conjunction with the Educational Coordinator.
  • Ensures compliance with all required DSS outcomes.
  • Coordinates quarterly shopping and purchase of additional items as necessary.
  • Complete weekly progress summary notes for residents on assigned caseload.
  • Provides appropriate documentation of resident services and maintains confidential and complete files for each resident.
  • Collaborates with the Social Worker and Independent Living Coordinator in the development and implementation of sustainable Independent Living Program for age appropriate residents.
  • Serves as the main point of contact with service agencies, including but not limited to: Department of Social Services, Department of Youth Services, Law Enforcement and Public Health.
  • Schedules conferences, meetings and visitations with agency representatives.
  • Intervenes, responds to, assesses and reports on critical incidents and emergencies. This may include assisting with de-escalation and passive restraint (Non Violent Crisis Intervention.)

Secondary Responsibilities:

  • Provides transportation and assistance as needed to carry out residential services.
  • Follows established reporting procedure for minor accidents, injuries, and behavioral issues.
  • Forms mutually respectful and trustful working relationships with team members.
  • Prepares statistical and narrative reports for Residential Services Director, Board of Directors and outside agencies as necessary.
  • Identifies and reports cottage maintenance needs to Facilities Coordinator.
  • Models appropriate team building skills/behavior to team members and serves as a role model for residents
  • Facilitates teammates in developing community resources for the benefit of the residents
  • Performs related duties as assigned.

Education and Training:

A Bachelor's Degree in Human Services from an accredited college or university and a minimum of 3 years experience in the population to be served. Prior experience with a children's service's-related group home or a similar organization is preferred but not required.

License:

A valid driver's license is required and must be maintained throughout employment.

Physical Demands and Working Conditions:

Must be physically able to operate a variety of office equipment including a computer, telephone, etc. Constant: standing. Frequent: repetitive use of hand, walking, standing, bending neck and waist, squatting, climbing, kneeling, twisting neck and waist, simple and power grasping of left and right hand, fine manipulation of left and right hand, reaching above shoulder level, reaching below shoulder level. Lifting objects up to 50 pounds from below the waist to a height of 3 feet. Restraining, with assistance, minors up to 18 years of age, weighing 150 pounds or more for up to 30 minutes. Occasionally running up to 75 feet over uneven ground. Must be able to exert up to twenty-five pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull or otherwise move objects or children.
